Visits start with a referral from primary care. During the visit, our team looks at health needs, medicines, memory, mood, daily tasks, and home safety. We coordinate with patient's primary care clinician to help guide care.
Caregivers play a key role in the assessment. We invite caregivers to join visits, share what they see at home, and help plan next steps. We also give advice and share tips to help at home.
Our help doesn't stop after the visit. Caregivers can join our support groups, learn ways to handle stress, and get help planning for changes like driving and long-term care. We stay in touch with the primary care team, so care stays clear and connected.
