You or your loved ones may notice changes in memory, mood, balance, or daily activities and feel unsure where to turn for guidance.
Our Center for Healthy Aging is an outpatient geriatric clinic with several locations throughout the region. We offer visits that help older adults and caregivers better understand concerns, reduce risks, and plan next steps for care and daily life.
Visits don't replace primary care. Instead, we work alongside your primary care team and share recommendations and findings to support ongoing care.

Before your visit
To get an assessment, ask for a referral from your primary care team. A referral may come after a wellness visit or a new concern.
Before your appointment, you’ll receive a questionnaire in the mail to complete before your visit. It asks about your goals, medications, daily activities, mood or behavior changes, sleep, falls, safety concerns, and advance care planning. Please bring the completed questionnaire to your assessment. Your care team will use this information to help guide the visit.
During your visit
Most visits take about 2 hours. During your visit, we’ll talk with you about your health, daily life, and any concerns or changes you’ve noticed. We’ll also make time to answer questions from you and your caregiver or loved one, if present.
During a geriatric assessment, our team may review:
- Activities of daily living and changes in independence
- Mood, behavior, and emotional health
- Memory loss or mild thinking changes
- Home and personal safety concerns
- Caregiver support needs and available resources
- Current medications, including whether they are appropriate for age and health needs and any possible side effects, drug interactions, or safety concerns
At the conclusion of your visit
Our team will review findings and discuss next steps to support healthy aging and daily life. You may receive:
- A review of testing and assessment results
- Recommendations for medication changes or ongoing care
- Suggestions to address home or personal safety concerns
- Lifestyle recommendations to support healthy aging
- Advance care planning discussions, including future care preferences and directives
We’ll share all of the information from your visit with you primary care team.
