Creating a culture of community health and wellness requires various cross-sector organizations working together. We value our partnerships and relationships throughout Southwest Virginia and rely on them to help shape our strategic direction and goals, provide financial support and offer grant opportunities.
To effectively execute Community Health Assessments, we partner with health assessment teams throughout Southwest Virginia. These teams oversee the health assessments; prioritize local barriers to optimal health; and work within our communities to create and implement strategies to reduce barriers, coordinate resources and enhance community strengths.
Carilion Clinic is committed to improving the health of the communities we serve by addressing key health priorities identified through our triennial Community Health Assessments (CHAs). We fulfill this commitment in many ways, one of which is through targeted grants for community health improvement programs.
Requests to fund health safety net programs will be reviewed twice annually. All completed applications must be received by April 15 for the spring funding cycle and/or October 15 for the fall funding cycle.
- Applications received by Oct. 15 will be reviewed for fall cycle funding (Jan. 1 - Dec. 31)
- Applications received by April 15 will be reviewed for spring cycle funding (July 1 - June 30)
We base decisions on funding community health improvement programs on available resources and the expected impact on documented community health needs. We require a written agreement and two progress reports. We may also schedule a site visit by Carilion Clinic Community Health and Outreach staff. In late summer 2021, Carilion Clinic will be posting the new 2021 Community Health Assessments (CHAs) that align with the service areas of each Carilion hospital. Starting with the October 2021 application, all requests for funding will be evaluated based on alignment with the new 2021 community health priorities identified in these assessments. Once posted, the 2021 CHAs and past CHAs can be found at https://carilionclinic.org/community-health-assessments.
