Peer Support Services and Community Health Workers

Community Health and Outreach is home to a team of staff ready to support community members in achieving their goals—whatever those might be. Our community health workers work one-on-one with clients to set goals and locate community resources. Our peer support specialists provide support through the mental health, substance use and trauma recovery processes.

Referrals
Carilion Clinic patients can be referred through Epic to Community Health and Outreach to enroll in group health education programs that support healthy lifestyles.
To place an order for group health education in Epic, complete the following:
- Type “AMB CHO” and hit enter
- Select code “REF7001” (AMB REFERRAL TO COMMUNITY HEALTH AND OUTREACH)
- In the list of options that appear, select Health Education and choose all that apply from the list below:
- Healthy Lifestyles
- Physical Activity
- Stress and Resilience
- Disease Management
- Other (enter free text)
- In the Notes section, type in a reason for referral

Grants

Carilion Clinic is committed to improving the health of the communities we serve by addressing key health priorities identified through our triennial Community Health Assessments (CHAs). We fulfill this commitment in many ways, one of which is through targeted grants for community health improvement programs.
UPDATE:
As part of our commitment to addressing key health priorities in our region, we currently review requests for targeted grants that support community health improvement programs twice annually.
This year, we are making changes to enhance the overall experience for both our grant committee and applicant organizations.
As part of this work, we will shift to a once-a-year grant process that will happen annually in the spring. Additional information on this new process will be shared in February 2026.
We host mandatory community grant applicant webinars prior to each grant cycle for organizations interested in applying. At least one organization representative must be present at the webinar to participate in the grant process.

Grant Applicant Notification
Sponsorships
All requests for community sponsorships must be received in writing at a minimum of six weeks prior to the date of the event or the recognition deadline, if applicable.
Application Criteria
- Organizations seeking funding must be 501(c)(3) not-for-profits
- Organizations must be located within or have a presence within Carilion Clinic's footprint
- Strong preference given to organizations supporting needs identified in the most recent Community Health Assessments (CHAs)
The request should include the following:
- Organization name and mailing address
- Federal tax identification number
- Contact name, title, phone number and e-mail address
- Name and date of the event or program
- Amount of request (please include all levels of sponsorships available)
- Details of event or program to be funded
- How your event or program will address issues identified through Carilion's CHAs
- Plans for recognizing funding organizations according to the sponsorship levels included
Email requests to community_sponsorships@carilionclinic.org. A sponsorship team meets monthly to review requests.
Exclusions
We do not sponsor the following:
- Religious organizations
- Political organizations
- Sports teams
- Requests for funds for individuals
- Endowments and memorials
- Individual school events, fundraisers, PTO/PTAs or booster clubs
- Organizations that discriminate by race, color, creed, gender, national origin or sexual orientation
Produce Prescription Program
What is a fruit and vegetable prescription program?
Poor diet is the leading cause of premature death and chronic diseases like heart disease and type 2 diabetes. For people with limited financial resources, managing diet-related chronic diseases can be difficult and lead to increased hospitalizations and higher healthcare costs.
A fruit and vegetable prescription program addresses these challenges by helping patients with diet-related health risks access nutritious foods. Healthcare professionals refer eligible patients to the program and prescriptions for fruits and vegetables are fulfilled through food retail or other food access sites. These programs are a promising “food is medicine” approach that can improve health equity and outcomes, optimize medical spend and increase patient engagement and satisfaction.
A 2019 study estimated implementing fruit and vegetable incentives for all Medicaid and Medicare beneficiaries could lead to significant health improvements and cost-savings, including:
- $39.7 billion saved in formal healthcare costs (over a lifetime)
- Almost 2 million cardiovascular events (e.g., stroke, heart attack) could be prevented
About our program:
Carilion’s SWPRx program is a 6-month intervention beginning annually in April/May that includes:
- Nutrition and healthy living classes (featuring Full Plate Living)
- Free fresh fruits and vegetables for the patient and their household (through Virginia Fresh Match outlets and Feeding Southwest Virginia)
SWPRx is funded by the Gus Schumacher Nutrition Incentive Program (GusNIP), from the USDA’s National Institute of Food and Agriculture (NIFA). GusNIP is a competitive NIFA grant program that prioritizes nutritional security by supporting projects across the food and healthcare system. The GusNIP Produce Prescription Program supports projects that demonstrate and evaluate the impact of fresh fruit and vegetable prescriptions to increase procurement and consumption of fruits and vegetables, reduce individual and household food insecurity, and reduce healthcare usage and associated costs.
Patients do not have to participate in research to be part of the program. However, all program participants will be invited to participate in research during their enrollment.
How do patients enroll?
To be eligible for this program, adult patients must receive Medicaid and have one of the following: elevated blood pressure, prediabetes or type 2 diabetes.
Eligible patients must be referred by their doctor. To place an order for the program in Epic, clinicians must complete the following:

Healthy Hearts Program
About this program:
Healthy Hearts is an evidence-based program created by the CDC and YMCA to help adults with hypertension lower their blood pressure. Participants receive a free blood pressure monitor and attend 8 10- to 15-minute personalized consultations with a trained coach for support with establishing a blood pressure self-monitoring routine. They also attend monthly group nutrition workshops that encourage heart-healthy diets and lifestyles.
To be eligible for the program, adult patients with hypertension must:
- NOT have had a heart attack, stroke or cardiac event in the last 12 months
- NOT have atrial fibrillation, arrhythmias or lymphedema
How do patients enroll?
Eligible patients can be referred to the program by their doctor or by calling Community Health and Outreach at 540-983-4053. To place an order for the program in Epic, clinicians must complete the following:
- Type “AMB CHO” and hit Enter
- “REF7001” will appear (AMB REFERRAL TO COMMUNITY HEALTH AND OUTREACH)
