Community Health Assessments

Our Community Health Assessments help us identify the strengths and barriers that impact health. The information we gather drives our programs, partnerships and investments.

Reports

Many organizations are dedicated to improving and maintaining a community’s health. We conduct these CHAs and make their findings public so health-oriented organizations have the information they need to plan programs and services that impact community health.

We produce these documents to benefit the community. We encourage the use of these reports for planning purposes—and we're interested in learning how they're used. We invite you to submit your comments and questions via email.
