CRMH Pharmacy Residency Training
Pharmacy Department
The Department of Pharmacy Services at Carilion Roanoke Memorial Hospital includes approximately 70 pharmacists and 70 technicians. We utilize a mix of decentralized clinical staff and specialists to provide pharmaceutical care for patient populations ranging from Level I trauma to neonatal critical care.
In addition to our personnel, we use multiple technologies to deliver care, including automated dispensing cabinets, centralized carousels, computerized prescriber order entry, electronic medical records, clinical decision support systems for surveillance and bedside barcode scanning. Approximately 45% of our pharmacists are adjunct or associate professors at regional schools of pharmacy and medicine.
CRMH is an academic medical center associated with the Virginia Tech Carilion School of Medicine, with numerous medical and surgical training programs as well as trainees from many other disciplines. Within this learning environment, our pharmacist staff are highly integrated into the care of our patients, participating in and leading clinical activities that include:

In addition to supporting the Pharmacy Residency Training programs, the department of pharmacy also operates an ASHP-Accredited Pharmacy Technician Training Program | Carilion Clinic.
PGY1 Pharmacy Residency

Thank you for your interest in our PGY1 Pharmacy Residency program. It was established in 2008 with our inaugural class. Since that time, we have grown our program to include 6 PGY1 residency positions. Our PGY1 program builds on doctor of pharmacy education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification and eligible for PGY2 Pharmacy residency training.
PGY1 Pharmacy residents will progress through a series of customized rotational and longitudinal learning experiences during the 12-month period. We offer a mix of required and elective learning experiences in multiple areas. Block rotations are one month long, which allows for 10 focused rotations (seven required and three elective).

PGY2 Critical Care Residency

The emphasis of our program is to develop pharmacists with expertise in critical care pharmacotherapy. Our graduates will be prepared to practice as clinical specialists in critical care and serve as preceptors for residents and students.
Residents will have the opportunity to continue to build their clinical and leadership skills, and to assist in precepting both students and PGY1 residents. PGY2 Infectious Diseases Residency

Thank you for your interest in our PGY2 Infectious Diseases Pharmacy Residency program.
We’re excited to confirm that we have early committed two residents into our Infectious Diseases program for 2026-2027.
The purpose of our PGY2 Infectious Diseases (ID) Pharmacy Residency program is to develop pharmacists adept in clinical management of infectious diseases and antimicrobial stewardship. Residents will be integrated as core members into an antimicrobial stewardship team of three ID clinical pharmacy specialists overseeing antimicrobial use at six hospitals and 200+ outpatient clinics.
The graduates of our PGY2 program will be prepared to practice as clinical specialists in infectious diseases, develop/manage antimicrobial stewardship programs and precept residents and students in infectious diseases pharmacotherapy.
Our two PGY2 ID Pharmacy residents will progress through a series of customized rotations and longitudinal learning experiences during the 12-month period. Our rotations are completed as a mix of focused, month-long blocks and longitudinal experiences.

PGY2 Internal Medicine Residency

Thank you for your interest in our PGY2 Internal Medicine Pharmacy Residency program, established in 2017.
We’re excited to confirm that we have early committed to a current PGY1 for our Internal Medicine program for 2026-2027.
PGY2 residency programs build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills and abilities as defined in the educational competency areas, goals and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.
Residents will have the opportunity to continue to build clinical and leadership skills, and to assist in precepting both students and PGY1 residents. Residents will progress through a series of customized, month-long, focused learning experiences in addition to longitudinal learning experiences over at least 52 weeks.

PGY1/PGY2 HSPAL Residency

PGY1 Pharmacy/PGY2 Health System Pharmacy Administration and Leadership Residency Program
Thank you for your interest in our combined PGY1 Pharmacy/PGY2 Health System Pharmacy Administration and Leadership (HSPAL) Residency program, established in 2020.
During the PGY1 year, the resident will matriculate through our well-established PGY1 residency program as our seventh PGY1 resident. Our PGY1 program builds on doctor of pharmacy education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions.
Following successful completion of the PGY1 residency requirements, the resident will transition into the PGY2 HSPAL.
The master of healthcare administration (MHA) is completed through Radford University Carilion (RUC). Master’s degree coursework is completed over four semesters beginning in August of the PGY1 residency year. The Department of Pharmacy works with leaders at RUC to complete the admissions process after a resident has matched with Carilion. The final number of credit hours needed to complete the MHA program is determined by the applicant’s pharmacy school coursework and allowable transfer hours as outlined by RUC.

The purpose of our PGY2 Emergency Medicine Pharmacy Residency program is to develop advanced clinical pharmacy practitioners with specialized expertise in emergency medicine pharmacotherapy. Our resident will work alongside a large, interdisciplinary team to enhance patient outcomes in a high-acuity, academic emergency department. This program aims to enhance the resident’s ability to provide high-quality, evidence-based pharmaceutical care in critical situations through direct patient care experiences, interdisciplinary collaboration, didactic learning and teaching, quality improvement initiatives and practice-based research. Graduates of this program will be prepared to serve as integral members of emergency medicine teams, precept pharmacy students and residents and attain specialty board certification.

Application Requirements
We participate in the PhORCAS (Pharmacy Online Residency Centralized Application Service) residency application process. PhORCAS is the result of a partnership between the American Society of Health-System Pharmacists (ASHP) and Liaison International. It also incorporates registration with the National Matching Service (NMS).
All candidates must be graduates of an Accreditation Council for Pharmacy Education (ACPE) accredited degree program (or be in the process of pursuing accreditation) or have a Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate from the National Association of Boards of Pharmacy (NABP). Applicants should also be eligible for Virginia licensure.
Additionally, interested PGY2 candidates shall have completed (or currently be completing) and ASHP-accredited PGY1 residency.
Applications for our residency programs require submission of the following information through PhORCAS:
- Letter of intent
- This letter must be personalized to Carilion and include answers to the following two questions.
- Why do you want to complete this residency (PGY1, PGY2, etc.)?
- Why do you want to complete your residency training at Carilion Roanoke Memorial Hospital?
- This letter must be personalized to Carilion and include answers to the following two questions.
- Curriculum vitae
- A minimum of 3 references submitted through PhORCAS (for PGY2 program applicants, we recommend one of the three references be from current residency program director)
- Academic transcripts
Completed applications are due by January 2nd each year.
