We offer a group of dedicated professionals with experience in end-of-life care. Our hospice care team includes nurses, doctors, nurse practitioners, certified nursing aides, board-certified music therapists, massage therapists, social workers, chaplains, bereavement counselors, and trained volunteers. They work together with the patient and family to support comfort, physical and emotional needs, and personal wishes.
Pain and symptom management
We provide personalized medical care to help manage pain, shortness of breath, nausea, anxiety, and other symptoms. We partner with the patient and family to promote comfort, quality of life, and peace of mind.
Emotional, spiritual, and practical support
Social workers, chaplains, and trained volunteers offer end-of-life resources, emotional support, spiritual care, and companionship. They remain involved throughout the care journey, helping to address practical needs along the way.
Caregiver support and guidance
We support caregivers in many ways, including listening to their concerns, teaching them what to expect, and providing resources. We also guide families through care choices, goals of care, and advance care planning. We're here to help with challenging situations and difficult decisions.
Bereavement services
We continue to support families after a loss. Grief support may include help with memorial planning, grief counseling, and support groups. Bereavement follow-up and support is offered for up to 13 months following the death of a loved one.

Pet Peace of Mind®
Pets can provide comfort and companionship during serious illness. Through Pet Peace of Mind, specially trained volunteers help patients care for their pets at home by assisting with daily needs and providing rehoming support when needed.
End-of-Life Doula Program
End-of-life doulas are specially trained hospice volunteers who provide companionship, emotional support, and guidance for patients and families. They use active listening, life review, and legacy work to help bring comfort, meaning, and connection during the final stages of life.
Partners in Care (PIC)
Partners in Care (PIC) is offered by Carilion Home Care and is specially designed to support patients with serious illness who may benefit from extra support but are not yet ready or eligible for hospice care.
