To ensure those who reside in a facility-based residence receive hospice care, Carilion Clinic Hospice holds established agreements with many assisted living and long-term care facilities. We partner together to create a plan of care that highlights the patient’s care needs, promotes their dignity, and ensures optimal pain and symptom management. We offer the same level of hospice care and support to someone living in a facility as to someone living in their home.
