What should you bring to your appointment?
First, fill out the application form attached (see button). It will ask for the following:
- A list of your current medications with strength and directions for taking them
- Proof of household income (pay stubs)
- A copy of your most recent tax return (if you file taxes)
- Your photo ID (a copy will be made)
- Any insurance cards you have (a copy will be made)
What can you expect to happen?
Your application may take some time to be approved. Once you are accepted into the program, our physicians will generally order your medications in a 90-day supply. Some medications are not available.
In most cases, medications will be delivered to your physician’s office and you will pick them up there. Medications that are picked up from a pharmacy may require a dispensing fee.
You are responsible for informing the MAP staff that you have received your medication. This is so your physician can reorder them properly. By cooperating with these rules, you help the MAP program remain successful.
