Your Carilion Clinic medical records are kept confidential and safe. To request physical copies of your medical records, you need to fill out a release form (see button) that gives us permission to send you this information. Please include a photocopy with picture identification (ID), such as your driver's license. The form and ID can be delivered in person, faxed or mailed to the locations noted below.
When received, your request will be submitted and your records will be mailed to you. This can take 1-2 weeks. A fee may be charged for this service and you may ask for a cost estimation. If you have questions, please contact your local Health Information Management (HIM) group or call your physician's office directly.
