Patient Resources
Our Diabetes Management Program provides a comprehensive approach to diabetes education and is recognized by the American Diabetes Association. Our team of Certified Diabetes Care & Education Specialists (CDCES) works closely with both adults and children to provide each patient with the tools necessary to manage their daily needs and live well with diabetes. A Certified Diabetes Care & Education Specialist is a health care professional who specializes in the management and prevention of diabetes. Our CDCES can partner with you to develop a plan and provide the knowledge, skills, and support you need to effectively manage diabetes.
We offer individual and group education sessions for patients with diabetes, pre-diabetes and gestational diabetes. Education can be provided in-person or via telehealth (video or phone) as needed.

For insurance purposes, we may need a referral from your primary care physician or endocrinologist. Our services are typically covered by Medicare, Medicaid, and most third party payors. Patients can also apply for Carilion financial assistance to help cover the cost of our program
