This team includes your primary care physician, advanced care practitioners (ACP), care coordinator, office nurse and off-site specialists, all working together to keep you and your family healthy.

The care coordinator’s role is to identify patients with chronic conditions, and through regular communication, based on your personal plan of care, help facilitate health improvements for a better quality of life.
