Additional Resources for Advance Care Planning
The Virginia State Bar website offers resources to help Virginia residents plan ahead for health care decisions related to end-of-life care and medical decision-making. Also, the Virginia Department of Health offers a secure registry for Virginia residents to store their Advanced Health Care Directive, Health Care Power of Attorney, Declaration of Anatomical Gift, and other documents so that medical providers, emergency responders, family members, and anyone else they grant access will honor their wishes.
