Carilion Clinic Research and Development requires numerous documentation forms throughout the timeline of a research project. These include:

  • R&D Project Checklist: First step in deciding what forms are required for your project.
  • Application for Research Projects QAQI_PHI: Required form; must be completed, with signatures, before R&D will approve application.
  • Clinical Trial Application: If the project is a Clinical Trial, this form will gather necessary information.
  • Cost Share Approval Form: Carilion Clinic will make a cost-sharing commitment only when mandated by the sponsor (required).  On rare occasions, Carilion Clinic will commit to cost-sharing due to the competitive nature of the award (voluntary).  All cost-sharing commitments must be reviewed and approved by the appropriate Department Chair and the VP for Academic Affairs before Research & Development will submit the application to the sponsor.
  • Proposal Routing and Approval Form: If the project involves funding, this form will gather the necessary information and approvals.
  • R&D Generic Service Request Form: The Service Request Form is used to request ancillary services required for a research protocol that would not be considered standard of care and therefore may be billable to the project.
  • Note: The Principal Investigator is responsible for completing only the top portion of the Service Request Form.
  • R&D Pharmacy IDS Service Request Form: The Service Request Form is used to request pharmacy services required for a research protocol that would not be considered standard of care and therefore may be billable to the project.

  • Preliminary Budget Worksheet: Aid for development of a budget. Please work with the R&D to complete your budget.
  • Statement_of_Work_SAMPLE: For study team members that are not employees of Carilion, please complete the Statement of Work and attach to your R&D Application.
