At the Carilion Clinic Heart Failure Clinic, we provide compassionate, comprehensive outpatient treatment for people with heart failure. With three convenient locations in Roanoke, New River Valley and Lexington, Virginia, we're here to help you to maximize the quality of your life and stay as healthy as possible. We provide a supportive atmosphere for you and your family to learn about your condition with the goal of helping you to avoid a visit to the emergency department or hospital admission.

Carilion's heart failure clinics are led by cardiologists who specialize in the diagnosis and management of both diastolic and systolic heart failure. Our program follows specific, evidenced-based guidelines from the American College of Cardiology, American Heart Association and Heart Failure Society of America.

At your initial visit, we'll take measurements of your weight, blood pressure, heart rate and level of oxygen in your blood. Your health history and medications will be reviewed. Other tests that may be required for the medical team to best help you may include chest X-rays, blood tests, EKGs, echocardiograms, sleep studies and/or six-minute walk tests. We'll work out a treatment plan regarding your plan of care and follow-up visits that is based on your specific needs. Your treatment plan will be sent to your primary care physician to keep him/her informed of your visit.

Some patients are referred to our outpatient cardiac rehabilitation program to learn more about how to manage their symptoms, as well as to improve nutrition and participate in safe, low-level exercise, if appropriate.

Carilion's Heart Failure Clinic is here to support you with the care you need to live well, including:

  • Early follow-up after discharge from the hospital
  • Ongoing visits to monitor signs and symptoms
  • Management of heart failure medications
  • Education and support to help manage the disease
  • Telephone consultations
  • Care coordination with the patient's primary physician
  • Referrals to other providers/services as needed, including Cardiac Rehabilitation for further education, management, and exercise as appropriate.
