Diabetes Management Program
A person with type 2 diabetes can use exercise to help control their blood sugar levels and provide energy their muscles need to function throughout the day.
Being diagnosed with diabetes can be overwhelming and frightening but an important thing to remember is that the more you know about diabetes, the more control you have. Here at Carilion Clinic, our Diabetes Management Program offers individual and group instruction that stresses self-management skills. Accredited by the American Diabetes Association since 1989, our program has helped many people take control of their diabetes.
Based on your personal medical history, our certified diabetes educators will help you set goals and will give you the tools and support you need to take charge of your health. Our nurses, registered dietitians and other instructors will help you learn about diet, exercise and other tools you can use to get healthy. We will tell you what to expect, how to monitor yourself and how to respond to changes and unexpected situations.
We provide comprehensive services to help keep you healthy, including:
An individual with diabetes should wear or carry I.D. information (such as an alert bracelet) that emergency medical staff can find. A sugar source, such as glucose tablets or raisins should be carried in case blood sugar levels become too low.

- Lifestyle management education, including instruction about diet and exercise
- Insulin pump education
Our registered dietitians can also provide medical nutritional therapy to help you learn how to make healthier food choices.
Diabetes is a chronic condition, so long-term care is important. We encourage you to regularly see your physicians for follow-up care. We communicate directly with your doctors so that your medical care is coordinated and complete. We also offer the full range of specialty services, such as vision care and wound care.
How to Enroll in Carilion Clinic’s Diabetes Management Program
The Diabetes Food Pyramid divides food into six groups, which vary in size to show relative amounts of servings for each group. The groups are based on protein content and carbohydrates instead of their food classification.
Your health care provider can refer you to our Diabetes Management Program. Medicare and many insurance carriers cover diabetes management services. We will bill your insurance upon completion of the course. You may be responsible for co-payment and/or the balance not covered by insurance. If you do not have insurance, let us know when you enroll - you may qualify for assistance.
We encourage you to include a family member or caretaker in the program, at no extra charge. The cost of the program includes group classes and individual sessions, snacks and materials used in the program, and follow-up services. We will be available by phone, e-mail, or through follow-up sessions to help you reach your goals. Our dedicated and specialized team of nurses, dietitians and certified diabetes educators are here to help you get off the diabetes roller coaster and live a full, happy and healthy life.
