Requesting Home Care Services


How Can I Request Home Care?

Home Care services must be ordered by a physician. Patients, families, or physicians may request home health services by calling 540-266-6000 or 800-964-9300.

A hospitalization is not required in order to receive home health care. Patients at home or those being treated by a physician may qualify.

Who Qualifies for Home Care?

Generally, patients qualify for home health if they:

  • Have a medical condition that requires the skills of a nurse or therapist to assess
  • Are under the care of a physician who will oversee their care
  • Have a medical condition that needs intermittent care and treatment
  • Must exert a taxing effort or require the assistance of others to leave the home

If you are hospitalized, your care team comprised of your physician and case manager or discharge planner will work together to arrange for your home care needs. They will provide the necessary information to help you select a home care provider.

Following your discharge, our clinician will contact you to schedule a convenient time to visit with you and initiate your home care.

The Cost of Your Care

Because home healthcare is a convenient, cost-saving alternative to hospital stays, it is covered under most private insurance plans, Medicare and Medicaid. Criteria for admission and reimbursement coverage vary. For your convenience, we will bill your insurance company directly.
