At Carilion Clinic, our physicians partner with patients and their families to deliver advanced primary care designed to keep them healthy and out of the hospital.
This type of healthcare is rooted in the "medical home" concept, part of Carilion Clinic's transition to a model where all healthcare providers are working towards the same goal: doing what's best for our patients. In a medical home, our physicians and staff take a proactive team approach to address patients' healthcare needs in a safe, timely manner.
For instance, a patient with diabetes may receive personal contact and health coaching from the practice's "care coordinator" to discuss blood sugar levels or test results. Patients also are encouraged to get involved in decisions about their care, which patients have embraced:

She took time with me one on one and that meant a lot. We reviewed some initial diet and exercise information, and then she set me up to see a dietitian in the Diabetes Management Program. He too, was very helpful, and together we made a plan that suited me. I am thrilled to say I have lost 15 lbs in the first month."

When you need specialty care, we will help you find the care you need and continue to manage your care even when other doctors are involved. Many of our family physicians do minor office surgery and deliver babies in addition to taking care of patients of all ages.
