Carilion Clinic Helps Diabetics Take Control of Lives
Many factors affect diabetes control. We can help you reach a balance through education. Our program stresses self management, which helps you set goals and learn the skills you will need to take charge of your health. Our diabetes program is always evolving to better meet the needs of patients and physicians. We provide a variety of comprehensive services to help keep you healthy, including:

Follow-up/Case Management
We know the importance of having a strong follow-up program for diabetes health. Our program is unlike any other in western Virginia because we understand the many factors involved with diabetes care. The program's comprehensive and unique features include one-on-one care, trained educators who are diabetes experts and direct communication with your doctor. Since we are based right here in your community 24 hours a day, our staff is knowledgeable about access to local resources. Patients receive consistent follow-up care through a personalized team approach.
How to Enroll
You will need a referral from your doctor or primary care provider to the Carilion Diabetes Management Program. We will ask your doctor for your medical history so we may serve you better. When you finish the program, we will send your doctor a summary of your activities with us.
Insurance Information
We will bill your insurance upon completion of the course. Medicare and many insurance carriers cover diabetes management services. You may be responsible for co-payment and/or the balance not covered by insurance, as appropriate. If you do not have insurance, let us know when you enroll — you may qualify for assistance.
We encourage you to include a family member or caretaker in the classes, at no extra charge. Program charges include group classes and individual sessions as appropriate, snacks and materials used in the program and one year of follow-up services. During the follow-up period, we will be available by phone or through follow-up classes to help you reach your goals.
